Blastocyst Transfer offers a way to eliminate high order multiple
gestations, while maintaining high clinical pregnancy rates.
A select group of patients may benefit from extending the time of
embryo culture to five or six days, especially those patients with
many embryos of roughly equal quality in which it is hard to choose
the best ones for transfer on day two or day three (namely; young
patients with many follicles and recipients of young donors).
A Blastocyst Transfer has many advantages. First, by extending the
length of culture to 5 or 6 days, we would support embryonic "Self
Selection", resulting in embryos with superior morphological quality
available for transfer, thereby transferring only best quality embryos.
Further limiting the number of embryos to transfer, this may reduce the number of high order multiple pregnancies.
The only concern for extended culture in vitro is the production of
large number of embryos exhibiting cleavage arrest or cleavage
abnormalities such as cell fragmentation, leading to the possibility
of failure of blastocyst development and resulting in the cancellation of
embryo transfer.
