In a bid to understand my chances of IVF success, I took a quick dive through the vast information available from these sources and came away thinking I had the information I needed. I skipped merrily along thinking things looked pretty promising after reading my chances of IVF working the first time was somewhere around the 40% mark. I naively thought that meant I had an 80% chance if I did two cycles, and that I’d definitely have a baby after three rounds at the most. Unfortunately as later reflection revealed, math and statistic just don’t work like this…
Although menopause is a natural barrier to further conception, IVF has allowed women to be pregnant in their fifties and sixties. Women whose uteruses have been appropriately prepared receive embryos that originated from an egg of an egg donor. Therefore, although these women do not have a genetic link with the child, they have a physical link through pregnancy and childbirth. In many cases the genetic father of the child is the woman's partner. Even after menopause the uterus is fully capable of carrying out a pregnancy.
Every woman is born with a set number of eggs, which declines as she ages. To get pregnant, an egg released from a woman’s ovaries must be fertilized by sperm, travel down the fallopian tube, and attach to the side of her uterus. At any stage along the way, a problem may occur, resulting in a case of infertility. For women, the most common causes of infertility are primary ovarian insufficiency (early menopause), ovulation disorders affecting egg release, uterine or cervical abnormalities, endometriosis (where tissue grows outside of the uterus), fallopian tube blockage or damage, polycystic ovary syndrome, and various hormonal imbalances. Certain cancers and their treatments can also negatively impact a woman’s fertility.
As with any medical procedure, there are some risks to keep in mind. When choosing between IUI and IVF, the risk is certainly something to consider. The chances of experiencing either a miscarriage or multiples are concerns many have when deciding to undergo fertility treatments. So let’s take a look at the odds of either of these things occurring, plus a few other risks to be aware of.
IVF is a type of assisted reproductive technology used for infertility treatment and gestational surrogacy. A fertilised egg may be implanted into a surrogate's uterus, and the resulting child is genetically unrelated to the surrogate. Some countries have banned or otherwise regulate the availability of IVF treatment, giving rise to fertility tourism. Restrictions on the availability of IVF include costs and age, in order for a woman to carry a healthy pregnancy to term. IVF is generally not used until less invasive or expensive options have failed or been determined unlikely to work.
A review in 2013 came to the result that infants resulting from IVF (with or without ICSI) have a relative risk of birth defects of 1.32 (95% confidence interval 1.24–1.42) compared to naturally conceived infants. In 2008, an analysis of the data of the National Birth Defects Study in the US found that certain birth defects were significantly more common in infants conceived through IVF, notably septal heart defects, cleft lip with or without cleft palate, esophageal atresia, and anorectal atresia; the mechanism of causality is unclear. However, in a population-wide cohort study of 308,974 births (with 6,163 using assisted reproductive technology and following children from birth to age five) researchers found: "The increased risk of birth defects associated with IVF was no longer significant after adjustment for parental factors."  Parental factors included known independent risks for birth defects such as maternal age, smoking status, etc. Multivariate correction did not remove the significance of the association of birth defects and ICSI (corrected odds ratio 1.57), although the authors speculate that underlying male infertility factors (which would be associated with the use of ICSI) may contribute to this observation and were not able to correct for these confounders. The authors also found that a history of infertility elevated risk itself in the absence of any treatment (odds ratio 1.29), consistent with a Danish national registry study  and "implicates patient factors in this increased risk." The authors of the Danish national registry study speculate: "our results suggest that the reported increased prevalence of congenital malformations seen in singletons born after assisted reproductive technology is partly due to the underlying infertility or its determinants."
With the exception of severe male infertility, 70% of the oocytes will become fertilized. In the case of severe male infertility, ICSI (pronounced ick-see) may be used to fertilize the eggs, instead of simply placing them in a culture dish. With ICSI, the embryologist will choose a healthy-looking sperm and inseminate the oocyte with the sperm using a special thin needle.
After your body releases an egg, the hormone progesterone kicks in to build and maintain the lining of the uterus. It makes your body temperature go up slightly. So taking your temperature with a basal thermometer every morning before you get out of bed can help you figure out if you ovulated. You can buy these thermometers at the drugstore. They're inexpensive, but they aren't as accurate as other ways of tracking ovulation.
At RMA, once the embryos reach the blastocyst stage, they are tested through a process called Preimplantation Genetic Testing for Aneuploidy (PGT-A), which lets doctors know which embryos have a normal number of chromosomes. While genetically normal embryos are much more likely to lead to pregnancy and healthy babies, the transfer of abnormal embryos will either result in no pregnancy, miscarriage, or an affected baby. While testing is occurring on a small part of the embryos, the embryos themselves are frozen, awaiting a receptive uterus. A large, prospective study performed recently at RMA confirmed that performing an embryo biopsy does not harm the embryo and does not decrease the likelihood of implantation.
After the retrieval procedure, you'll be kept for a few hours to make sure all is well. Light spotting is common, as well as lower abdominal cramping, but most feel better in a day or so after the procedure. You'll also be told to watch for signs of ovarian hyperstimulation syndrome, a side effect from fertility drug use during IVF treatment in 10% of patients.

In December 2015, the Ontario provincial government enacted the Ontario Fertility Program for patients with medical and non-medical infertility, regardless of sexual orientation, gender or family composition. Eligible patients for IVF treatment must be Ontario residents under the age of 43 and have a valid Ontario Health Insurance Plan card and have not already undergone any IVF cycles. Coverage is extensive, but not universal. Coverage extends to certain blood and urine tests, physician/nurse counselling and consultations, certain ultrasounds, up to two cycle monitorings, embryo thawing, freezing and culture, fertilisation and embryology services, single transfers of all embryos, and one surgical sperm retrieval using certain techniques only if necessary. Drugs and medications are not covered under this Program, along with psychologist or social worker counselling, storage and shipping of eggs, sperm or embryos, and the purchase of donor sperm or eggs.
When you know your menstrual cycle, you improve your chances of getting pregnant. The first phase starts with the first day of bleeding during your period. Your body releases hormones, like follicle-stimulating hormone (FSH), that make the eggs inside your ovaries grow. Between days 2 and 14, those hormones also help thicken the lining of your uterus to get ready for a fertilized egg. This is called the follicular stage.
