Endometriosis implants are most commonly found on the ovaries, the Fallopian tubes, outer surfaces of the uterus or intestines, and on the surface lining of the pelvic cavity. They also can be found in the vagina, cervix, and bladder. Endometriosis may not produce any symptoms, but when it does the most common symptom is pelvic pain that worsens just prior to menstruation and improves at the end of the menstrual period. Other symptoms of endometriosis include pain during sex, pain with pelvic examinations, cramping or pain during bowel movements or urination, and infertility. Treatment of endometriosis can be with medication or surgery.

Time. The factor of time cuts both ways when you are weighing up these two options. IUI is a much shorter process than IVF, so if your first cycle is successful, it could be the quickest route to bringing home a baby. However, because of the gap in success rates between the two treatments, some patients in their late 30s to early 40s may get pregnant faster by going directly to IVF rather than waiting until they have had several failed IUI cycles.
Ovulation induction (in the sense of medical treatment aiming for the development of one or two ovulatory follicles) is an alternative for women with anovulation or oligoovulation, since it is less expensive and more easy to control. It generally involves antiestrogens such as clomifene citrate or letrozole, and is followed by natural or artificial insemination.
With egg donation and IVF, women who are past their reproductive years, have infertile male partners, have idiopathic female-fertility issues, or have reached menopause can still become pregnant. Adriana Iliescu held the record as the oldest woman to give birth using IVF and a donor egg, when she gave birth in 2004 at the age of 66, a record passed in 2006. After the IVF treatment some couples are able to get pregnant without any fertility treatments. In 2018 it was estimated that eight million children had been born worldwide using IVF and other assisted reproduction techniques.
For cases where donor sperm is required, the majority of patients use commercial sperm banks as their source of donor sperm. You can find out a lot about a potential donor before making your choice. For example, you can access information about the donor’s ethnic or racial background, education, and physical characteristics. It’s also reassuring to know that sperm banks always screen donors for sexually transmitted diseases (STDs) such as chlamydia, syphilis, gonorrhea, hepatitis, HIV, and others. The FDA and the Centers for Disease Control and Prevention (CDC) recommend that frozen semen from a licensed sperm bank be used in order to prevent STDs.
At the same time, in older women, the IVF success rates can vary dramatically, and that’s why it’s so important to focus only on live births. For example, a clinic may have a very high pregnancy rate among older women, but a low live birth rate. Or, the rates may be quite high – 40% or even 50% – but only after four or five rounds. That makes a very big difference, especially in the overall cost of treatment!
Assess your preconception prep. Have you been on top of the preconception game or are you just too busy for baby-planning activities like charting and timing baby-making sex (or any sex for that matter)? Given that you have a little one underfoot, it's understandable if you're more exhausted than ever. It’s not easy for wannabe second-time parents to devote as much time and energy to TTC as they likely did on the first go-around, but it would be helpful to take a step back (and a hard look) at what's going on. Are your cycles still regular, or have there been any changes that might be hurting your chances for conception success? Have you been able to pinpoint ovulation with accuracy, or are you just having sex whenever (which would make conception less likely)? Getting back on track with tracking your fertility signs may be enough to put you back in the game.

Success rates for IVF depend on a number of factors, including the reason for infertility, where you're having the procedure done, and your age. Antiphospholipid syndrome (phospholipid antibody syndrome or Hughes syndrome) is an immune system disorder with symptoms that include: excessive blood clotting, miscarriages unexplained fetal death, or premature birth. In antiphospholipid syndrome, these symptoms are accompanied by the presence of antiphospholipid antibodies (cardiolipin or lupus anticoagulant antibodies) in the blood. Treatment focuses on preventing clotting by thinning the blood with the use of anticoagulants and aspirin.
Embryo donation is the least expensive of the donor options. It's often cheaper than a regular IVF cycle. An embryo donor cycle costs anywhere between $5,000 and $7,000. This is assuming the embryo has already been created. (As opposed to choosing an egg donor and sperm donor and having the embryo created specifically for your cycle, which would be extremely expensive.)
