Help is out there, if you want it, and takes many forms. West stresses the importance of investigating both the women and the men, "even if they have previously had a healthy sperm analysis because situations and lifestyles can change". There is also the alternative therapy route: acupuncture, hypnotherapy, reflexology, meditation. Or, if all else fails, you could, like me, go for in-vitro fertilisation (IVF).
Success rates for IVF depend on a number of factors, including the reason for infertility, where you're having the procedure done, and your age. The CDC compiles national statistics for all assisted reproductive technology (ART) procedures performed in the U.S., including IVF, GIFT, and ZIFT, although IVF is by far the most common; it accounts for 99% of the procedures. In the laboratory, for ICSI treatments, the identified eggs are stripped of surrounding cells (also known as cumulus cells) and prepared for fertilisation. An oocyte selection may be performed prior to fertilisation to select eggs that can be fertilized, as it is required they are in metaphase II. There are cases in which if oocytes are in the metaphase I stage, they can be kept being cultured so as to undergo a posterior sperm injection. In the meantime, semen is prepared for fertilisation by removing inactive cells and seminal fluid in a process called sperm washing. If semen is being provided by a sperm donor, it will usually have been prepared for treatment before being frozen and quarantined, and it will be thawed ready for use.

At the same time, in older women, the IVF success rates can vary dramatically, and that’s why it’s so important to focus only on live births. For example, a clinic may have a very high pregnancy rate among older women, but a low live birth rate. Or, the rates may be quite high – 40% or even 50% – but only after four or five rounds. That makes a very big difference, especially in the overall cost of treatment!
Amongst these two patient populations (which we’ll refer to now as PCOS), IUI with clomid and letrozole generally shows no improvement versus when these drugs are taken without IUI. Other health related problems could also cause poor egg health, low ovarian reserve, or abnormal immunological responses, which can affect conception. Stress could also play a role. We all know that menstrual cycles can be altered during times of extreme duress- and this can be emotional, physical, or environmental stressors. In these instances, the first steps should be to avoid life stressors, maintain a healthy weight, routinely exercise, avoid smoking, and reduce alcohol intake, all of which may be contributing to unexplained infertility issues.
Israel has the highest rate of IVF in the world, with 1657 procedures performed per million people per year. Couples without children can receive funding for IVF for up to two children. The same funding is available for women without children who will raise up to 2 children in a single parent home. IVF is available for women aged 18 to 45. The Israeli Health Ministry says it spends roughly $3450 per procedure.
Twenty-eight days is the average length of a menstrual cycle, though anything between 21 and 35 days is considered normal. Fluctuating a little from month to month is one thing, but if your period is so irregular that you don’t even try to track it anymore, it could indicate a problem producing eggs, or ovulating. Ovulation disorders (meaning you ovulate infrequently or not at all) account for infertility in about 25 percent of infertile couples, according to the Mayo Clinic. One of the most common causes of female infertility is polycystic ovarian syndrome (PCOS)—a condition characterized by longer than normal stretches between periods, or even skipping cycles for months in a row. Irregular periods may also result from excessive physical or emotional stress, which can mess with the hormones responsible for stimulating ovulation each month; being too heavy or too thin, or gaining or losing a lot of weight quickly may also have the same effect. Talk to your doctor; he may be able to prescribe fertility drugs to help induce or stimulate ovulation.

Though there are some risk with older women pregnancies, there are some benefits associated with caesareans. A study has shown that births over 40 have a lower rate of birth trauma due to increased delivery by caesarean. Though caesarean is seen to benefit mothers over 40, there are still many risk factors to consider. Caesarean section may be a risk in the same way that gestational diabetes is.
